The licensee failed to correctly record the required acquisition entries for 5 percent or more of the licensee’s total acquisitions. The licensee also failed to record any ID on less than five percent of Forms 4473 examined. The recommendation rose to a Warning Conference based on the licensee's inspection history of having a previous warning letter and warning conference for similar violations. Based on the licensee's improvement and willingness to institute additional procedures to ensure accuracy, IOI redacted recommends an alternate recommendation of a Violations, Warning Letter Only and Recall Inspection . IOI redacted also recommends release of the Special Attention Flag (SAF) and issuance of the renewal license.
The Area Supervisor concurs with the lOI's recommendation of a WL & Recall inspection instead of a WC .examined. The recommendation rose to a Warning Conference based on the licensee's inspection history of having a previous warning letter and warning conference for similar violations.
