Received 12/2/14 from Ernie Litner: Attached is a log entry pertaining to Lighthouse Gunsmiths LLC in Fairfield, OH which indicates the transfer of a firearm prior to the 3 business day wait. The creation date for this NTN was 11/15 and the MDI Date would be 11/20. 4473 attached indicating transfer on 11/19. This NTN has been referred to CE as a retrieval. The NTN was not determined to be a denial until 12/01 so the early transfer did not cause the retrieval. See spreadsheets for info.
On October 13, 2015, Industry Operations Investigators ( IOI ) redacted conducted an unannounced Firearms Recall Compliance inspection at Lighthouse Gunsmiths, LLC. The inspection covered the period of 10/13/14 to 10/13/15. The inspection disclosed the following: Seven violations cited: Those violations included: 1) 27 CFR 478.124(c)(1), on redacted; 2) 27 CR 478.124(c)(3)(i), on redacted forms; 3) 27 CFR 478.124 (c)(3)(iii), on redacted forms; 4) 27 CFR 478.123 (c)(3)(iv), on redacted forms; 5) 27 CFR 478.124 (c)(5), on redacted form; 6) 27 CFR 478.99(c), on redacted form, and 7) 27 CFR 478.126a, on redacted forms. Further inspection disclosed 299 firearms on hand, 692 forms 4473 on file/reviewed, and 90 NTNs provided by FBI- NICS within the last 90 days. See narrative report.
Alternate recommendation from a Warning Conferece to Warning Letter and Recall for FY17. As a result of licensee’s improved business operations since his last inspection, and the fact that licensee was able to retrieve the NTNs from his spiral notebook, it is recommended that an Alternate Warning Letter be issued and a recall inspection conducted oppose to a Warning Conference being held.
