Report of Violations, Form 5030.5, issued 5/13/16, for violations of 27 CFR §§ 478.21(a), 478.99(c), 478.124(c)(1), and 478.125(e). It is recommended that a Warning Letter be forwarded to this licensee for these violations. It is further recommended that the renewal application be approved and license renewed with the amended address of: 177 S. Centre Street, Unit F, Merchantville, NJ 08109.
