VIOLS , WL Only and No Recall Inspection recommended ILO of Warning Conference. An alternate recommendation of Warning Letter and No Recall is submitted in lieu of recommending Warning Conference for 2 reasons: 1) The inspection did not disclose any other violations which would warrant a Warning Conference. 2) The violations disclosed were not deemed as being purposeful and/or willful . The internal controls that have now been established by the Licensee are more than adequate to ensure compliance, but it is the undersigned's contention that a refresher was needed. Mr. Terry’s sincere willingness to learn from the disclosed violations was evident by this IOI .
Due to the licensee's limited compliance history and the nature of the violations, the DIO concurs with the assigned lOI's recommendation for a warning letter.
